Making a gift in your will or with a beneficiary designation?

For bequest language, the following is suggested:

“I give, devise, and bequeath to Regional One Health Foundation (EIN 58-1737037), a nonprofit organization located at 877 Jefferson Ave., Memphis, TN 38103, or its successor thereto, ____________________ [specific dollar amount/specific percentage of the estate, determined at the date of my death/the rest, residue, and remainder of my estate after my obligations are met/description of personal property/all the right, title, and interest in and to the real estate located at address or description of property], to be used to meet the most urgent needs.”

For a contingent bequest, the following language is suggested:

“If [primary beneficiary] does not survive me, then I hereby give, devise, and bequeath to Regional One Health Foundation (EIN 58-1737037), a nonprofit organization located at 877 Jefferson Ave., Memphis, TN 38103, or its successor thereto, [description of the property] to be used to meet the most urgent needs.”

Life Insurance Gifts

To name Regional One Health Foundation as the beneficiary of a life insurance policy, you may request a Change of Beneficiary form from your policy administrator or download the form from your insurer’s website. Add the following information to correctly identify Regional One Health Foundation as the beneficiary and return the form to establish your gift:

  • Legal Name: Regional One Health Foundation
  • Tax ID Number: 58-1737037
  • Address: 877 Jefferson Ave., Memphis, TN 38103

Here are a few options for donating a life insurance policy.

  1. 1. Transfer Ownership of an Existing Policy:You can give an existing paid-up policy to Regional One Health Foundation, which can qualify for a charitable income tax deduction (if you itemize), equal to the policy’s replacement value or basis, whichever is less. Your agent/broker can help facilitate the change of ownership, or you may contact the carrier directly to transfer the existing policy.
  2. 2. Name Regional One Health Foundation as a Beneficiary:You may make a charitable beneficiary designation naming Regional One Health Foundation as either:
    • The primary beneficiary,
    • A percentage beneficiary, along with one or more beneficiaries, or
    • A contingent beneficiary, to receive the proceeds only if the primary beneficiary cannot.

If you need assistance with updating or changing the beneficiary, you can seek help directly from the agent, broker, or carrier, who can facilitate the beneficiary designation update or change.

  1. 3. Create a New Policy in Regional One Health Foundation’s name: You can establish a new life insurance policy with Regional One Health Foundation as the owner and beneficiary if there’s an insurance interest. The premium payments may qualify as tax-deductible contributions, depending on the circumstances. Contact us to determine whether this is an option for you.
  2. 4. Transfer a Paid-Up Policy into a CRT: Transferring a paid-up policy into a charitable remainder trust (CRT) can provide many tax and financial benefits. The trust will pay an income to your beneficiaries for life. When the trust beneficiary dies, the remaining trust assets pass directly to Regional One Health Foundation. You receive an immediate income tax deduction when you establish the trust for the present value of our remainder interest. To ensure the transfer is handled correctly and provides the intended benefits, we highly recommend consulting a qualified attorney.
